attempt to seek outside of the file error

hey guys. im getting the 'attempt to seek outside of the file' error in qemu and i have no idea what it means.
just reads like some cryptic message that an old man would tell you at 3 am right before you become the main character of an anime
the error seems to be from grub though but apart from that i have no idea what causes it
sorry if this question is very dumb but i have no idea what causes it ive searched everywhere by now
