USB 2.0 and USB hub support working in RDOS

Finally it seems like the USB stack for 2.0 (including hub-support) works as it should. It's been a long way, and there still are some minor problems with certain chipsets (AMD) where the interrupt IN pipes malfunctions. Tested successfully with hubs, HID devices, USB-to-serial converters and USB printer. Also works with companion controllers (OHCI and UHCI).
