Hi,
AFAIK there isn't one. Ralf brown's interrupt list (google it) contains a similar list somewhere though, IIRC.
Generally knowing what device you want to access helps you narrow down searches though.
Cheers,
James

Be careful looking for Ralf Brown's list though... He seems to have gotten quite unusual advertising - some may be NSFW.
Also, once you reach things like hard disks or network cards, you can use the PCI bus (assuming of course the card is on the PCI bus and not on ISA) to find the port bases (among other things).

Many of the current devices do not have fixed addresses anymore. Once you've gotten to the basic PC/AT hardware, you're pretty much done, as the remaining stuff is pretty much indeterminate.
That aside, what do you intend to use that list for. I can't think of a good reason why you'd want to link an unknown port number to a device - Either you already have a driver for it, or it's "advanced" stuff you don't need to care about yet.

That's just the first link Google gives you, at
http://www.ctyme.com/rbrown.htm, but luckily the NSFW adds are below your average screen height, so just don't scroll down :).
