File Systems

How important is the filesystem choice that you make? For instance would OS X be an awful OS if it used FAT 12..? what are the pro's and cons to designing your own file system? I just wanna lay a simple FS down so i can move on, ive been using FAT12 but my OS is getn better and better and im ready for somthin thats gonna be permanent.
