Where I can find some well-written documents telling me the address of the output and input ports?

Umm if you're just looking for the basics (keyboard, PIT, etc.) look around for HelpPC.

Is HelpPC a book or something else? excuse me I'm asking this question!

Wow, that map's way better, but if you still want HelpPC (it's a programmer's reference tool for dos, but I bet you could recompile it for whatever os you want), you can get it at http://www.cs.nyu.edu/~yap/classes/machineOrg/helppc/
