computing the page directory and page table entries

Whew! It's working now. The final problem had to do with how I was getting the address of the page table entry for the page directory entry.

I've checked in the current changes and will merge them to the master branch.

I cannot even begin to thank those of you who helped me with this enough. I could not have done this on my own.
