How would I start making an OpenBSD distribution?

I'd suggest Linux from Scratch first. It could help before doing OpenBSD

I'm not 100% on OpenBSD, but with FreeBSD the userland and kernel source come together.
You might be able to find the source of both in /usr/src and then just make install it.

You might peruse https://distrowatch.com/ for samples of other opennsd distributions to see how they did it... They also have a Comment section which appears to be their simple forum for asking questions.
