Makefile tutorial

Over 7 months after I started a rewrite and said I would continue "tonight", I actually got around finishing that rewrite.

Mistakes have been removed, improvements from the PDCLib Makefile been merged into the tutorial, and the overall structure of the document cleaned up a bit.

Those of you who based their Makefile on the tutorial should review the changes.
Every good solution is obvious once you've found it.
